Graduate School Forms for Staff and Faculty

Acceptance forms are to be sent to students admitted with aid and must be returned by them to the admitting department. Fellowship and Graduate (Teaching) Assistant Appointments are submitted by Departments to the Dean's office and then forwarded to the Graduate School.
